Above Ceiling Inspection Form

by Brad Keyes, CHSP, on Dec 17, 2020 1:00:00 AM

Q: I am looking for an Above Ceiling Inspection Form; something I can give to my Facilities guys that tells them what to look for when doing an above ceiling inspection (i.e., penetrations, wires on sprinkler piping, etc.). Would you have one that you can share?

A: Yes, I do. I sent you a new form that I made that includes many items to look for above the ceiling, such as:

  • Rated barriers
  • Gypsum patches
  • Embedded pipes
  • Fire-stop materials
  • Structural steel fire-coating
  • Suspended ceiling tiles
  • Water damage
  • Sprinkler pipe and hangers
  • Medical gas pipe
  • Electrical wires, cable, and cords
  • Electrical conduit & junctions boxes

This new form also includes NFPA code references as well.

If any reader would like a free copy of this new Above Ceiling Inspection Form, please send a request to and put "Above Ceiling Inspection Form" in the subject box.
